Care Management differs from standard practice as it links together a multi component treatment plan using elements from many different organizations. During this course, you identify patients in need of CM from a pool using a copyrighted tool based on state of the art research. You then use the VSM tool to create a current state map for a specific patient treatment pathway. 

Next, the current state is analyzed, and non value added activities identified. Then a future state map is constructed that eliminates those activities. Eco Mapping is used to identify the local resources available, and the process of setting up the relationships necessary to incorporate them into the treatment plan are explored. An implementation plan is developed including communication methods and strategies and producing a streamlined operating flow. 

Attendees will emerge with an understanding of Care Management systems and processes, a network of contacts and references to call upon when the time comes to make those possibilities a reality, and access to BR’s learning network and online learning resources
